If you would like a prescription refilled, please fill out the form

IMPORTANT NOTE!!
By Law, we cannot refill a prescription unless your pet has been
examined by one of our doctors within the last year. If your
pet has not been seen in the past year, please call us to make an
appointment for an office visit.
Please give us at least one business day (Monday-Friday) to have the
Pharmacy Tech, Jeff, get the doctor's approval to refill and to
make sure we have your prescription in stock. Jeff will call you
when the prescription has been filled and is ready for pickup. |
